I/We, the undersigned parent(s) or legal guardian(s) of the minor participant named above, acknowledge that participation in any Quarterback Magazine, LLC youth camp or related activities involves inherent risks, including but not limited to serious injury, disability, or death. I/We voluntarily allow the participant to take part in these activities and assume all such risks. On behalf of myself/ourselves, the participant, and our heirs, assigns, and representatives, I/We hereby release, waive, and discharge Quarterback Magazine, LLC, its owners, directors, coaches, staff, volunteers, affiliates, and agents from any and all liability, claims, demands, or causes of action arising from injury, loss, or damage connected to participation, except for gross negligence or intentional misconduct by the released parties. I/We agree that the participant will follow all rules and instructions, authorize emergency medical treatment if needed, confirm the participant is in good health with no conditions that would prevent safe participation, and verify that we carry adequate health insurance to cover any medical expenses.
